Those are the stats un-gemmed. I don't know how to obtain it. As for when it was introduced, it was never formally introduced.

it was given to the top something players on all servers, most players don't use it and it stays in there vault

I didn't hear many complaints. It's +25 atk, which is 42 atk when fully gemmed. If we made it any better we'd just hear complaints on how it's TOO good..

nope, reforged is just a touch less atk then holy blade.
